In 2007, sales of branded atypical
antipsychotics generated $15.9 billion in ex-manufacturer sales in the seven
major markets. In the United States alone, branded atypicals totaled $12.3
billion in ex-manufacturer sales. In the seven major markets, the atypical
antipsychotics class is led by Janssen’s Risperdal (risperidone), which
generated U.S. ex-manufacturer sales of $4.3 billion in 2007. In June 2008,
this market will undergo major upheaval when Risperdal loses market exclusivity
in the United States.
Decision Resources’ Physician&Payer
Forum survey "The Impact of Risperdal’s Patent Expiry on the Antipsychotic
Market: Clinician and Payer Attitudes" examines the effect that the advent of
major generic competition will have on future prescribing patterns and
reimbursement policies. We surveyed 84 psychiatrists, 70 primary care
physicians, 20 managed care organization (MCO) pharmacy directors (PDs), and 16
state Medicaid PDs and compared their responses to assess similarities and
differences. This survey enabled Decision Resources to do the following:
- Determine the magnitude of the impact of
generic risperidone on the first- and second-line use of branded antipsychotics
across indications and physician specialties.
- Determine the success of Janssen’s recently
launched atypical, Invega: clinicians’ desired attributes, current use, and
formulary coverage and restrictions.
- Analyze the current formulary tier
assignments of antipsychotics based on plan type (including Medicare, Medicaid,
and private plans) and review the level of copayments associated with each tier
and the cost controls imposed by agent and plan type.
- Analyze the future tier assignments of
antipsychotics and the expectations of MCO PDs concerning generic substitution,
therapeutic interchange, and various cost-control approaches they expect to
introduce following the availability of generic risperidone.
- Determine the impact of Risperdal’s patent
expiry on state Medicaid reimbursement of branded antipsychotics.
- Understand the factors influencing
formulary inclusion and analyze the future tier assignments of emerging agents
fulfilling a range of profiles.
- Uncover alignments and misalignments in the
attitudes and behaviors of clinicians versus MCO and state Medicaid PDs in
factors that will influence the uptake of these agents.
