  Introduction:

Half of men and two-thirds of women who die suddenly of coronary heart disease (CHD) have no prior symptoms; their subclinical atherosclerosis is largely undiagnosed and untreated. The most urgent need in this market is prevention of vulnerable atherosclerotic plaques, which represents a considerable growth opportunity through 2014.

  Questions Answered in This Report:

The blockbuster statins atorvastatin (Pfizer’s Lipitor) and simvastatin (Merck’s Zocor) face competition from generic statins during our forecast period. Will single-pill combinations of atorvastatin or simvastatin with adjunctive therapies protect the franchises of these branded agents as generic statins gain approval?

Currently, use of clopidogrel (Sanofi-Aventis/Bristol-Myers Squibb’s Plavix/Iscover) is confined to PMI patients undergoing percutaneous coronary intervention (PCI) and to long-term use in aspirin-intolerant patients. However, new data from the CHARISMA trial are expected in 2006. Will clopidogrel be able to build on its strong evidence base and achieve greater usage? Or will it face strong competition from emerging antiplatelet agents?

The EUROPA trial showed that long-term use of perindopril (Servier’s Coversyl) decreased cardiovascular morbidity and mortality and extended the knowledge acquired from the earlier HOPE trial of ramipril (Wyeth/Monarch/King’s Altace, Sanofi-Aventis’s Tritace). Will Servier be able to capitalize on these favorable results in CHD patients? Or will there be a general increase in angiotensin-converting enzyme inhibitor (ACEI) use?
